Pilocystic astrocytoma. Case contributed by . rof Frank Gaillard ◉ ◈. Diagnosis certain. Sections show a variably cellular tumor with a fibrillary background. Tumor cells have elongated bipolar to round nuclei with inconspicuous nucleoli. Frequent Rosenthal fibers are seen. There are focal aggregates of thin-walled, hyalinised blood vessels with the adjacent parenchyma containing hemosiderin deposition consistent with a cyst wall. No mitoses, necrosis or microvascular proliferation are identified.
Pilocytic astrocytoma gets its name because the tumor originates from star-shaped cells in the brain called astrocytes. Astrocytes are glial cells, which help to protect and sustain brain cells called neurons. Tumors that arise from glial cells are collectively referred to as gliomas. A pilocytic astrocytoma is most commonly found in a part of the brain called the cerebellum. They can also occur near the brainstem, in the cerebrum, near the optic nerve, or in the hypothalamic region of the brain. The tumor is typically slow-growing and doesn’t spread. That is, it’s considered benign.
Pilocytic astrocytoma is a relatively well-circumscribed astrocytoma that occurs most frequently in children and young adults with a peak incidence in the teens. 1 Pilocytic astrocytomas occur sporadically and in association with neurofibromatosis type 1 (NF1). Pilocytic astrocytoma exhibits a paucity of consistent genetic abnormalities predictive of outcome. 49 It is, however, interesting that these tumors may display abnormalities on chromosome 17 in the same region as the neurofibromatosis type I (NFI) gene, and patients with NFI have a higher incidence of pilocytic astrocytomas. 67 Pilocytic astrocytomas may spread locally, and very uncommonly they have been found to spread. Pilocytic astrocytoma or juvenile pilocytic astrocytoma or cystic cerebellar astrocytoma (and its variant juvenile pilomyxoid astrocytoma) is a brain tumor that occurs more often in children and young adults (in the first 20 years of life). They usually arise in the cerebellum, near the brainstem, in the hypothalamic region, or the optic chiasm, but they may occur in any area where astrocytes are present, including the cerebral hemispheres and the spinal cord.
