Apparently, this was the first rendition of the Maiden Japan, but the manager didn't approve. This is probably because vocalist Paul Di'Anno (pictured as a severed head) was actually in the process of being kicked from the band. Of course, this was kept a secret from Paul, and the manager felt that this artwork would be seen in bad taste once Paul was let go after the Maiden Japan tour.
